El seu d\u00e8ficit pot donar lloc a debilitat i fragilitat \u00f2ssia, el que anomenem osteomal\u00e0cia, i conseq\u00fcentment, a fractures dels ossos.<\/p>\n<h2>Quanta vitamina D necessito?<\/h2>\n<p>Segons l&#8217;Institute of Medicine (IOM) es recomana consumir 600 unitats internacionals (UI) (20 mcg) de vitamina D al dia en adults de fins a 70 anys, nens i embarassades. En el cas dels majors de 70 anys, la recomanaci\u00f3 ascendeix fins a 800 UI di\u00e0ries.<\/p>\n<h2>Quines s\u00f3n les fonts de vitamina D?<\/h2>\n<p>Aquesta vitamina <strong>es troba de forma natural en molt pocs aliments<\/strong> (com ara el peix blau, els ous i els xampinyons) i l&#8217;obtenim principalment gr\u00e0cies a la <strong>s\u00edntesi que es produeix a nivell de la pell amb l&#8217;exposici\u00f3 solar<\/strong>. Despr\u00e9s passar\u00e0 pel fetge per convertir-se en 25-OH-vitamina D i finalment anir\u00e0 al rony\u00f3, que s&#8217;encarrega de fabricar la seva forma activa, la 1,25-OH-vitamina D.<\/p>\n<h2>Pot ser que em falti vitamina D?<\/h2>\n<p>El <strong>d\u00e8ficit de vitamina D \u00e9s molt freq\u00fcent<\/strong> (fins a un 70% de la poblaci\u00f3 segons un estudi realitzat entre el 2000 i el 2004), i dep\u00e8n sobretot de l&#8217;exposici\u00f3 solar. Altres factors que afavoreixen aquesta situaci\u00f3 s\u00f3n l&#8217;envelliment, l&#8217;obesitat, alguns f\u00e0rmacs (per exemple la colestiramina), o b\u00e9 presentar problemes malabsortius o insufici\u00e8ncia renal o hep\u00e0tica.<\/p>\n<h2>Qu\u00e8 sentir\u00e9 si em falta vitamina D?<\/h2>\n<p>En general, el d\u00e8ficit de vitamina D \u00e9s <strong>asimptom\u00e0tic<\/strong>. Tot i aix\u00ed, els pacients amb un d\u00e8ficit prolongat i sever poden presentar <strong>desmineralitzaci\u00f3 \u00f2ssia i osteomal\u00e0cia<\/strong>. De vegades tamb\u00e9 pot apar\u00e8ixer dolor ossi, debilitat muscular i fractures. De moment no hi ha prou dades per relacionar la manca de vitamina D amb malalties que no siguin de l&#8217;os.<\/p>\n<h2>I llavors, com es diagnostica?<\/h2>\n<p>Per saber si hi ha d\u00e8ficit de vitamina D \u00e9s necess\u00e0ria una <strong>anal\u00edtica sangu\u00ednia que inclogui 25-OH-vitamina D<\/strong>. Es recomana sol\u00b7licitar-la en aquells pacients que es troben en els grups de risc que hem explicat i el nivell de refer\u00e8ncia \u00e9s 30-50 ng \/ ml (75-125 nmol \/ L). Si la vitamina D es troba per sota de 10 ng \/ ml parlem de d\u00e8ficit, per sota de 30 ng \/ ml es defineix com insufici\u00e8ncia.<\/p>\n<h2>Qu\u00e8 he de fer si tinc d\u00e8ficit de vitamina D?<\/h2>\n<p>En cas de vitamina D insuficient o deficient es recomana <strong>valoraci\u00f3 per un metge especialista<\/strong> per decidir si necessita <strong>suplements<\/strong> i en quina dosi.<\/p>\n<p>En el servei d&#8217;Endocrinologia de Cl\u00ednica\u00a0ServiDigest, podem realitzar el diagn\u00f2stic del d\u00e8ficit de vitamina D, valorar suplementaci\u00f3 mitjan\u00e7ant dieta o complements farmacol\u00f2gics, fer el seguiment posterior i descartar malalties \u00f2ssies associades.<\/p>\n<p>Dra.
