{"id":40093,"date":"2024-05-20T22:39:34","date_gmt":"2024-05-20T20:39:34","guid":{"rendered":"https:\/\/dev.93-93-113-41.plesk.page\/?page_id=40093"},"modified":"2024-05-24T14:16:00","modified_gmt":"2024-05-24T12:16:00","slug":"sindrome-metabolica","status":"publish","type":"page","link":"https:\/\/dev.93-93-113-41.plesk.page\/ca\/serveis\/endocrinologia-i-nutricio\/sindrome-metabolica\/","title":{"rendered":"S\u00edndrome metab\u00f2lica"},"content":{"rendered":"\n<p>La s\u00edndrome metab\u00f2lica (SM) \u00e9s el conjunt de factors de risc que augmenten exponencialment les probabilitats de sofrir malalties cardiovasculars, diabetis i en conseq\u00fc\u00e8ncia augment de la mortalitat.<\/p>\n\n\n\n<p>Entre ells es troben l\u2019obesitat, la hipertensi\u00f3 arterial, hipergluc\u00e8mia, i nivells baixos de colesterol de baixa densitat (HDL), comunament conegut com a \u2018colesterol bo\u2019.<\/p>\n\n\n\n<p>Al llarg dels anys han sorgit diferents definicions i criteris per a diagnosticar aquesta s\u00edndrome, per\u00f2 tots coincideixen que \u00e9s el conjunt de les anteriors alteracions les que el componen.<\/p>\n\n\n\n<p>Segons National Cholesterol Education Program Adult Treatment Panell III (NCEP-ATP-III) s\u2019estableix el diagn\u00f2stic per la pres\u00e8ncia d\u2019almenys tres dels seg\u00fcents criteris: obesitat usant l\u2019IMC \u2265 30 kg\/m\u00b2; triglic\u00e8rids (TG) \u2265 150 mg\/dl o en tractament; colesterol HDL &lt; 40 mg\/dl per a homes o &lt; 50 mg\/dl per a dones; pressi\u00f3 arterial sist\u00f2lica \u2265 130 mmHg, i diast\u00f2lica \u2265 85 mmHg; i\/o tractament antihipertensiu i determinaci\u00f3 de gluc\u00e8mia en dej\u00fa \u2265 100 mg\/dl o en tractament diab\u00e8tic.<\/p>\n\n\n\n<p>S\u00edndrome Metab\u00f2lica = Suma de tres dels seg\u00fcents criteris<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Obesitat<\/li>\n\n\n\n<li>Triglic\u00e8rids elevats<\/li>\n\n\n\n<li>Baixos nivells de Colesterol HDL (colesterol bo)<\/li>\n\n\n\n<li>Hipertensi\u00f3 arterial<\/li>\n\n\n\n<li>Hipergluc\u00e8mia <\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Obesitat<\/h2>\n\n\n\n<p>L\u2019organitzaci\u00f3 mundial de la salut defineix el sobrep\u00e8s i l\u2019obesitat com \u201duna acumulaci\u00f3 anormal o excessiva de greix que pot ser perjudicial per a la salut\u201d.<\/p>\n\n\n\n<p>Patir obesitat \u00e9s un factor de risc per a sofrir malalties cardiovasculars i diabetis de tipus 2.<\/p>\n\n\n\n<p>Hi ha diferents criteris per a diagnosticar l\u2019obesitat, el m\u00e9s utilitzat \u00e9s l\u2019\u00edndex de massa corporal (IMC) que es calcula mitjan\u00e7ant l\u2019altura i el pes.<\/p>\n\n\n\n<p>Una altra f\u00f3rmula \u00e9s l\u2019\u00edndex de cintura \u2013 maluc (ICC) que es calcula mitjan\u00e7ant el per\u00edmetre de la cintura i el maluc (ICC).<\/p>\n\n\n\n<p>De totes dues f\u00f3rmules obtenim un valor num\u00e8ric que ens situa dins d\u2019uns par\u00e0metres que determinen si estem en estat saludable o no. Un percentatge de greix corporal superior al 33% en homes i al 25% en dones tamb\u00e9 es considera obesitat.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">[IMC = pes en kg \/ (al\u00e7ada en cm) 2]<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Infra pes = &lt; 18.5<\/li>\n\n\n\n<li>Normo pes = 18.5 \u2013 24.9<\/li>\n\n\n\n<li>Sobrep\u00e8s = 25 \u2013 29.9<\/li>\n\n\n\n<li>Obesitat = > 30<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">[ICC = per\u00edmetre de la cintura en cm \/ per\u00edmetre del maluc en cm]<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Valores normales en dones = 0.71 \u2013 0.84<\/li>\n\n\n\n<li>Valores normales en hombes = 0.78 \u2013 0.94<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Triglic\u00e8rids elevats<\/h2>\n\n\n\n<p>Els triglic\u00e8rids (TG) s\u00f3n un tipus de greix que circula pel corrent sanguini. El cos els utilitza per a emmagatzemar l\u2019energia que ingerim en la dieta i no consumim. Una dieta en la qual hi ha un exc\u00e9s de greixos i\/o hidrats de carboni provocar\u00e0 un augment dels TG. Un nivell elevat de TG \u00e9s un factor de risc per a sofrir malalties cardiovasculars ja que contribueix a la formaci\u00f3 de la capa ateromatosa en les art\u00e8ries (capa que es forma a partir de l\u2019exc\u00e9s de colesterol i TG podent comprometre el reg sanguini en les art\u00e8ries). <\/p>\n\n\n\n<p>Podem con\u00e8ixer els nostres nivells de TG mitjan\u00e7ant una an\u00e0lisi de sang.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Nivells de Triglic\u00e8rids en sang<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Normal = &lt; 150 mg\/dL<\/li>\n\n\n\n<li>L\u00edmit alt = 150 a 199 mg\/dL<\/li>\n\n\n\n<li>Alt = 200 a 499 mg\/dL<\/li>\n\n\n\n<li>Molt alt = 500 mg\/dL o superior<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Baixos nivells de Colesterol d\u2019alta densitat (HDL)<\/h2>\n\n\n\n<p>El colesterol d\u2019alta densitat (HDL sigles en angl\u00e8s) \u00e9s comunament conegut com el colesterol bo. Aquest nom se li atribueix pel fet que \u00e9s l\u2019encarregat de transportar el colesterol de baixa densitat (LDL sigles en angl\u00e8s), conegut com a colesterol dolent, al fetge per a ser eliminat. Un exc\u00e9s de LDL \u00e9s un factor de risc cardiovascular ja que, igual que els TG, s\u2019acumula en les art\u00e8ries formant la capa ateromatosa. Per tant, \u00e9s important tenir uns bons nivells de HDL per a evitar la formaci\u00f3 d\u2019aquesta placa. Per a aconseguir-ho, s\u2019han de limitar en la dieta les fonts de greixos saturats i trans i potenciar el consum de greixos insaturats (peix blau, alvocat, oli d\u2019oliva verge extra, fruita seca), a m\u00e9s d\u2019evitar el tabac. <\/p>\n\n\n\n<p>Podem con\u00e8ixer els nostres nivells de HDL mitjan\u00e7ant una an\u00e0lisi de sang.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Nivells de HDL en sang<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Nivells \u00f2ptims en dones = > 50 mg\/dl<\/li>\n\n\n\n<li>Nivells \u00f2ptims en homes = > 40 mg\/dl<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Hipertensi\u00f3 arterial<\/h2>\n\n\n\n<p>La hipertensi\u00f3 arterial (HTA) \u00e9s l\u2019augment de la tensi\u00f3 que exerceix la sang en les art\u00e8ries quan \u00e9s bombada pel cor. La HTA mantinguda en el temps \u00e9s un factor de risc per a sofrir malalties cardiovasculars.<\/p>\n\n\n\n<p>Els factors externs que poden causar HTA s\u00f3n: una dieta poc saludable que faciliti desenvolupar sobrep\u00e8s o obesitat, exc\u00e9s de sodi o d\u00e8ficit de potassi en la dieta, el tabac, la inactivitat, l\u2019estr\u00e8s i alcohol.<\/p>\n\n\n\n<p>Podem mesurar els nostres nivells de tensi\u00f3 arterial mitjan\u00e7ant un tensi\u00f2metre.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Nivell de Tensi\u00f3 Arterial Normal<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sist\u00f2lica = &lt; 120 i Diast\u00f2lica &lt; 80<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Hipergluc\u00e8mia<\/h2>\n\n\n\n<p>Els nivells elevats de glucosa (sucre) en sang es coneixen com a hipergluc\u00e8mia.<\/p>\n\n\n\n<p>Es considera que els nivells estan augmentats quan l\u2019hemoglobina glicosilada (HbA1c) est\u00e0 per sobre del 5,7%. Aquest valor indica com han estat els nivells de glucosa en els \u00faltims 3 mesos i a partir de 6,4% ja es considera diabetis de tipus 2 (DM2).<\/p>\n\n\n\n<p>Si no hi ha cap patologia pr\u00e8via, els nivells de glucosa en sang poden veure\u2019s augmentats per l\u2019obesitat, la falta d\u2019activitat f\u00edsica, una mala alimentaci\u00f3, exc\u00e9s de carbohidrats i sucres en la dieta, el tabac i l\u2019alcohol.<\/p>\n\n\n\n<p>Tenir hipergluc\u00e8mia i no controlar-la \u00e9s l\u2019avantsala per a sofrir diabetis de tipus 2. Les alteracions descrites anteriorment, per si soles, ja suposen un factor de risc per a sofrir malalties cardiovasculars i\/o DM2. La s\u00edndrome metab\u00f2lica en ser el conjunt d\u2019aquestes, suposa un augment considerable del risc a sofrir aquestes malalties.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">S\u00edndrome Metab\u00f2lica i H\u00e0bits Saludables<\/h2>\n\n\n\n<p>L\u2019estil de vida i els h\u00e0bits saludables s\u00f3n una condici\u00f3 primordial per a combatre la s\u00edndrome metab\u00f2lica. Seguir una dieta saludable amb patr\u00f3 de dieta mediterr\u00e0nia, mantenir un pes saludable, fer activitat f\u00edsica i evitar els h\u00e0bits t\u00f2xics com l\u2019alcohol i el tabac s\u00f3n la primera l\u00ednia de defensa contra la s\u00edndrome metab\u00f2lica i les seves complicacions.<\/p>\n\n\n\n<p>Per a complir amb el patr\u00f3 de dieta mediterr\u00e0nia, s\u2019han de consumir 5 racions de fruites i verdures al llarg del dia. Els cereals com l\u2019arr\u00f2s, la pasta, el pa\u2026 han de ser integrals i evitar les farines refinades i els sucres sempre que sigui possible. Una altra font de fibra important s\u00f3n els llegums que es recomana consumir-les un m\u00ednim de 2 vegades per setmana.<\/p>\n\n\n\n<p>Mantenir un consum moderat d\u2019hidrats de carboni, aix\u00ed com evitar els sucres afegits en la dieta, s\u00f3n factors determinants per a mantenir uns nivells correctes de glucosa en sang i evitar l\u2019augment de TG i colesterol LDL. A m\u00e9s, una dieta rica en fibra afavoreix mantenir valors normals de TG i colesterol tant LDL, com HDL.<\/p>\n\n\n\n<p>El peix blau es recomana consumir com a m\u00ednim 3 vegades per setmana, la fruita seca i l\u2019oli d\u2019oliva verge extra, s\u00f3n font de greixos saludables que afavoreixen l\u2019augment del colesterol HDL. A part de consumir greixos saludables, s\u2019ha de remarcar la import\u00e0ncia de limitar al m\u00e0xim, els greixos trans i saturats provinents d\u2019aliments processats com a embotits, brioixeria i carns vermelles\u2026 que potencien l\u2019augment de LDL i TG. D\u2019altra banda, el consum de sal tamb\u00e9 s\u2019ha de tenir present, ja que un exc\u00e9s de sodi pot afavorir l\u2019augment de la tensi\u00f3 arterial. \u00c9s recomanable limitar el consum de sal de taula i utilitzar altres saboritzants com a esp\u00e8cies, llimona, i herbes arom\u00e0tiques, aix\u00ed com deixar els aliments m\u00e9s rics en sodi per al consum ocasional (marisc, embotits, formatges curats\u2026).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Dieta Mediterr\u00e0nia \u2013 Freq\u00fc\u00e8ncia de consum<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">Consumir:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>5 racions entre fruites i verdures di\u00e0riament<\/li>\n\n\n\n<li>Cereals integrals (evitar refinats i sucres afegits)<\/li>\n\n\n\n<li>Fruita seca<\/li>\n\n\n\n<li>Oli d\u2019oliva verge extra<\/li>\n\n\n\n<li>Peix blau 3\/setmana<\/li>\n\n\n\n<li>Llegums 2\/setmana<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Evitar:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Processats<\/li>\n\n\n\n<li>Carns vermelles o grasses (limitar el consum a 1 vegada per setmana)<\/li>\n\n\n\n<li>Sucres afegits o productes que els continguin <\/li>\n\n\n\n<li>Alcohol<\/li>\n<\/ul>\n\n\n\n<p>La realitzaci\u00f3 d\u2019activitat f\u00edsica \u00e9s una arma molt potent contra la s\u00edndrome metab\u00f2lica, ja que ajudar\u00e0 a millorar el perfil lip\u00eddic (TG, LDL) i els nivells de tensi\u00f3 arterial. Aix\u00ed mateix, afavoreix la utilitzaci\u00f3 de glucosa per part del m\u00fascul esquel\u00e8tic ajudant a reduir els nivells de sucre en sang i a mantenir un pes saludable consumint les reserves d\u2019energia que tenim emmagatzemades en forma de greix o evitant que s\u2019acumuli.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Dieta Mediterr\u00e0nia \u2013 H\u00e0bits saludables<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Realitzar activitat f\u00edsica di\u00e0riament \u2192 >10.000 passos +3-5 hores \/ setmana d\u2019activitat moderada o 75 minuts \/ setmana d\u2019activitat intensa.<\/li>\n\n\n\n<li>No fumar<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Factors de Risc modificables en la S\u00edndrome Metab\u00f2lica<\/h2>\n\n\n\n<p>S\u2019ha de tenir en compte que existeixen factors de risc que no s\u00f3n modificables, \u00e9s a dir que no podem fer res per canviar-los, com serien l\u2019edat, la gen\u00e8tica, el sexe, els antecedents familiars etc.<\/p>\n\n\n\n<p>D\u2019altra banda, hi ha molts altres que s\u00ed que ho s\u00f3n, la dieta, l\u2019exercici, evitar els h\u00e0bits t\u00f2xics.
